Optimal Care, Inc. provides the highest quality care homecare by practicing under an advance practice model of care. Our model incorporates advance practice nurses in leadership, quality assurance and direct patient care. Advance practice nurses are educationally prepared at the post-graduate level and provide a high degree of knowledge, skill and experience to achieve optimal outcomes for our patients. Our advance practice nurses utilize extended and expanded skills in assessment, planning, implementation, and evaluation in caring for our patients. Within the nurse patient relationship advance practice nurses use critical analysis, problem solving and evidence based decision making in caring for our most complex patients. Our clinical leaders use their expertise to develop therapeutic and caring relationships to support and mentor our clinical staff. Our quality leaders use their substantial education to ensure our care is the highest care possible using evidence-based practice guidelines to improve nursing practice, patient outcomes and overall collaboration with the physician overseeing the plan of care.

Geriatric Care
As caring professionals, we understand the needs of older adults. Our clinicians have advanced knowledge and expertise in geriatrics and develop each plan of care to address the physical, emotional and spiritual needs of our patients.
Geriatric Nursing Care
Our advance practice model of care allows us to provide the highest level of nursing care tailored to meet the unique needs of older adults. Nurse practitioners review every patient’s plan of care to ensure the most up to date evidence based care is being provided. Many of our patients have complex medical conditions that require the high level of problem solving and interventions offered by our nurse practitioners. To help our patients achieve optimal outcomes our nurse practitioners make consultation visits and often become the primary nurse case managing our patients in our geriatric program. To support our patients after hours a nurse practitioner is always available on call to help patient, families and community partners problem solve and make clinical decisions.
Geriatric Therapy
Our physical, occupational and speech therapists have extensive experience working with the geriatric population. Our therapists receive additional training, evidence-based assessments and interventions specific to geriatrics. Our training includes unique balance, gait, activities of daily living and cognition.
