Medical social workers help patients and their families deal with social, emotional and environmental issues.

SPECIFIC MEDICAL SOCIAL WORKER SERVICES:

Services provided by qualified medical social workers include but are not limited to:

• Psychosocial assessment and reassessment of the client
• Referral to community resources as needed
• Nursing home or institutional placement assistance as needed
• Providing psychotherapy to the client and family as indicated
• Conferring with patient, family and team members regarding social, emotional, and environmental issues relating to the client’s health problems
• Documentation of the initial assessment, periodic evaluations, progress notes and other documentation for the clinical record
• Consulting with the physician regarding the client’s status if his/her condition is adversely affected by social, environmental or economic conditions
• Development, implementation, review and revision of the plan of care
• Participation in case conferences, staff meetings, and inservice programs as indicated
