Professional services of PHYSICAL THERAPISTS from OPTIMAL CARE, INC.

Physical Therapists evaluate neuromuscular and functional ability and assist with regaining muscle strength, coordination, balance, walking and function.

SPECIFIC PHYSICAL THERAPY SERVICES:

Provide physical therapy services by qualified licensed physical therapists in accordance with the physician’s plan of treatment including but not limited to:

  • Assessment and reassessment of the client and assistance in evaluating the client by administering muscle, nerve, joint and functional ability tests
  • Directing and assisting clients in active and passive exercises, muscle reeducation, and functional training, activities of daily living, including transfer activities and prosthetic training.
  • Observing, recording and reporting to the physician the client’s reaction to treatment and any change in the client’s condition
  • Instructing the client in the care and use of wheelchairs, braces, crutches, canes and prosthetic and orthotic devices
  • Instructing other health team members, including home health aides and family members, in the home program
  • Development, implementation, review and revision of the plan of care and documenting clinical and progress notes, summaries, and other clinical record information
  • Participation in case conferences, staff meeting, and inservice programs as indicated
  • Supervision of the physical therapy assistant and home health aide
  • Treating clients to relieve pain, develop or restore function, and maintain maximum performance by using teaching, exercise, massage and applicable therapy treatments and modalities
