Professional services of REGISTERED NURSES offered by OPTIMAL CARE, INC.

Registered Nurses provide skilled nursing care in the home under a physcian’s directive. The nurse communicates with the physician to update or change the patient’s plan of care, discusses prevention strategies and coordinates patient care and services with the physician and other health team members.

SPECIFIC SKILLED NURSING SERVICES:
Provide skilled nursing services by registered nurses in accordance with the physician’s plan of treatment, including but are not limited to:

  • Assessment and regular reassessment of the nursing needs of the client throughout the course of care
  • Development, implementation, review and revision of the nursing care plan
  • Providing nursing services including treatments, diagnostic and preventative procedures
  • Inititiation of preventative and rehabilitative nursing procedures as appropriate for the client’s care and safety
  • Observation of physical and emotional signs and symptoms and reporting to the physician of all reactions to treatments, including medication administration
  • Teaching, supervising and counseling of the client and family regarding the nursing care needs and related problems of the client at home
  • Supervision and teaching of other nursing service personnel such as home health aides
  • Coordination of total client care
  • Notification of physician of client progress
  • Assisting the physician in the development/renewal of the plan of treatment
  • Preparation of progress notes, summary reports, medication profiles, discharge summaries, and other documentation
  • Participation in case conferences, staff meetings, and inservice programs as required by OPTIMAL CARE, INC.
