Professional services of of SPEECH LANGUAGE THERAPISTS at OPTIMAL CARE, INC.:

Speech therapists assist with restoration of language skills, ability to swallow, and hearing or memory problems. They also teach other providers and family members how to help patients develop communication skills.

SPECIFIC SPEECH THERAPIST SERVICES:

Speech Therapy services by a qualified speech pathologist under the supervision of a qualified clinical supervisor will be performed in accordance with the physician’s plan of treatment and can include but not be limited to:

• Assessment and reassessment of the client and evaluation and testing of clients with speech, hearing or language disorders
• Assisting the physician in determining and recommending appropriate speech and hearing services and in development and renewal of the physician’s plan of treatment
• Providing rehabilitative services for speech, hearing and language disorders
• Recording and reporting to the physician the client’s reaction to treatment and any changes in the client’s condition
• Instructing other health team and family members in the home rehabilitation program
• Development, implementation, review and revision of the plan of care and documenting clinical and progress notes, summaries, and other documentation for the clinical record
• Participation in case conferences, staff meetings, discharge planning activities and in service programs as indicated.
